Silly me said yes to a double shift at SSH. On Casual pay. That means it's not counted as overtime for me. Setting that aside you think I would have learnt from past experience that anytime you get begged for a double it means that it's going to be a shit day cause their super down on numbers.


In Mac
The morning is a blur of ladies who had her midwifery things sorted and were just waiting for the doc to either do an ultrasound and send them on their merry way or make a decision so they don't have to return for more unnecessary appointments in MAC because midwives can't make decisions.

Until the senior's lady needed a cannula (IVC) and me and my keenbeans to get some experience was like "I'll do it" then started to try (and she wasn't easy btw.) using a trolley with a sharps container that was full (mistake one), failed the first IVC. Couldn't put it in the sharps bin (full) forgot to retract the needle in with the retractor (mistake two) and put it down on the sterile field to try again. Got the second one in- went to get the bung from the field - and badly stuck myself with IVC number 1.

Sighs.

Her antenatal bloods are all negative for HIV and hepatitis-- and her only dodgy history is she's from South East Asia- (no IVDU hx or the like) but still here we go.

So I had to tell the boss. Was in ED for ages. Got sent to look for "staff health" only to find out that SSH doesn't have staff health, we share the staff health at MSH and the intern that was dealing with me had no idea what to do, phooned staff health, got the MSH staff health who didn't realise he was at SSH- until I phoned Staff health and asked: "so where are you guys because I'm at E block at SSH and no one here is saying their you" and they are reply: "Oh. Your at SSH...."

Anyhow.

I heard the manager saying she got the woman to agree to repeat blood test -
I could have just gone in there and taken it from her myself.
But I didn't because I felt shitty. I'm kicking myself now because I'm fairly sure it's not been done.
Course the last time our manager did anything clinical was probably pre-historic so she probably didn't do it.

My test is not going to show if I got anything: things take time before that sort of transmission shows.

But the woman retests as negative, then there would be nothing to get. Other than diseases that aren't commonly tested for.

Meanwhile I later find out that the procedure we were rushing to get her ready for: and then she ended up just waiting in the side room for : (fasting mind you, with that IVC in) - then got cancelled because it was ferrel busy - and sent home to return tomorrow for it - so someone would have taken the not-required IVC out.

I just want her results more than anything. Cause 0.000001% chance of her being positive is still more than the 0 chance that I was negative, yesterday. And this means P and I gotta stop trying until those results come back because if he got sick, because I got sick, from a mistake - would make the mistake so much more worse.

booking in BMI 41, GDM diet controlled.
Palpating her was like palpating a waterbed that is 20% empty. I could not tell you what position that kid was in.
She came in - clearly labouring - and the senior said she was 6cm and fully effaced.
1 hour of me fluffing around getting a cannula in, trying to obtain a decent CTG, giving antibiotics (that only half the bag ran in) - she told her husband she was starting to push - and literally just as I was changing my gloves (that was the mistake) to get the birth trolley out (luckily someone already set it up because I probably wouldn't have in my fluffing) -the head was descending and by the time I stuck my head in the hallway and was like we're pushing in here! She had her gloves on already and literally caught the head like she was diving for a tennis ball.

Kid's out. Who needs the IVABs now? or the CTG. APGARS 9,9.

Paperwork pile commenced.

As the nightstaff was coming on, all the seniors were complaining about "why are none of the staff restocking their birth trolley as soon as they're done cleaning because -- if we need another trolley, it should always be ready to go. And I was thinking that's because I prioritised other things. For me, it was paperwork because I was at the birth, I could do the paperwork much faster than anyone else can do it. So that's what I prioritised. The other junior (both of us bitching in the store room) well I prioritised doing the shower so the nightstaff wouldn't have to do it.

We did the trolleys in 5 mins and I left.

On my way out I realised I should check that boss had gotten the blood from needle stick lady - and from what I could tell on the chart system, she hadn't. And that lady has gone home now, to return tomorrow for her procedure.
