Last week, I took N12X3 shifts cause I really didn't feel well/reliable.

So I returned on Monday figuring I couldn't just very well take the next 8 months off - and it was a little foggy and shitty.

Day 2 (today) of the week tends to be worse than day 1 but actually; it was pretty sweet.

I had this very cute old Punjabi lady; day 1 post liver transplant - who could speak very broken English, better, if you had the accent.

We extubated her because although she didn't obey commands you could still have half a conversation with her about pulling out (or not) pulling out the tube.

Then they dilly-dallied all day about if she needed a vascath: deciding so at 17:00 and then ordering platelet cover for the line insertion only to realise on ordering it that it wouldn't be ready until 18:30.

Started running it at super fast speed at 18:45 to start putting in the line at 19:00 (and I'm clock watching here - cause we all finish at 19:30 ) and worried she's gonna start talking in Punjabi again about God knows what she needs (ice, her religious scarf thing it turns out, and all manner of things) Lady the doc has a massive needle he wants to put in your L) internal jugular vein... please stay still...

20mcg bolus of fentanyl sorted that out and we were all good. The insertion was textbook and scarily bloodless (compared to other blood baths I'd seen) - I had prepared endless bluey cover for the new sheet and even guarded the back of the bed against drips with an old sheet.

Turns out not needed. (Better than cleaning of blood from between the railings or, even worse, when it's gotten into the bed's wheel.

When I left, a RN who could speak enough of her language to get a basic conversation going was going to be there that night. I was like Damn if only you were here during the day ....

But anyways.

It was a good day.

Highlight of the day: I made friends with a storeman a few weeks back by complimenting him on his Hawaii shirt. (you know: oh have you been to Hawaii? It's nice isn't it... blah blah small talk) and he told me today, that a staff member over in the other building had a + covid result necessitating shutting OT and other staff needing to get tested. Dunno if it's a true story, but it was nice that someone bothered to tell me the latest gossip from elsewhere in the giant machine that is MICH).
